LIQUID CHEMICALS IN DRINKING WATER CONTAINER

LIQUID CHEMICALS IN DRINKING WATER CONTAINER

At any construction site, a drinking water bottle or container has become one of our best friend. You can notice that almost 60 - 80% of the workers at construction site are carrying a drinking water bottle with them.

But there is one unsafe practice or issue is going at site and it's still happening up to now. This is when a certain worker will use a drinking water bottle for another purpose and then another worker with the same bottle will use it for drinking.

One scenario is that a liquid chemical or thinner (which its physical appearance is almost the same of water) was filled in a drinking water bottle and by mistakenly a certain worker without his/her knowledge that the liquid inside the bottle is a thinner, will consumed it or use for drinking. This example scenario is very common incident happening at construction site.

HOW TO STOP OR PREVENT THIS UNSAFE PRACTICE?

1.      All liquid chemicals must be kept or stored on their respective containers.
2.       If it was used with another container, provide a proper identification label. Do not keep the liquid chemicals on an unlabeled containers.
3.       Do not use drinking bottles for any other purposes.
4.       Assigned a trained or competent person at site to monitor and control hazardous chemical substances.
5.       Conduct toolbox talks and communicate to all workforce on this regard.

Following this five (5) basic good practice, will eliminate the issue of chemicals in drinking water bottles.

UNSAFE EXCAVATED AREA

As shown on the photo above, the excavated area has a depth of approximately 4 m. An open excavation presents a dangerous situation. Here, the foreman in-charge failed to carry out the necessary safety measures after completing its tasks, leaving the area in an unsafe condition without proper barricading and signage.

Potential risk that may lead into accident:

Person or heavy equipment falling into excavation and getting seriously injured or damaged.

Preventive actions taken:

The safety team observed the potential hazards and instructed to close out outstanding items such as providing proper barricading, signage and rectification works to the area immediately before allowing any activity to commence on the said area.

Lessons Learned:

  1. All excavated areas must be attended to safely before and after the work.
  2. A safe work procedure must be carried out to prevent accidents.
  3. Traffic Management/Logistic Plan shall be in place also and suitable and sufficient barrier for the risk such as pedestrian or vehicle barriers must be placed.
  4. Deep excavation signs must be placed with preferred language ensuring that everybody can understand.
  5. Access in and out of the excavation must be placed so as not to impede on traffic rules nor into a dangerous situation.

EDGE PROTECTION FOR OPEN ELEVATOR OR LIFT SHAFT AT BUILDING CONSTRUCTION

Building construction sites are very dangerous and risky place to work. Accidents/Incident and even death may happen at construction sites anytime but using our common sense mostly of these hazards are preventable.

Let's discuss about one major accident happening at site is the Fall From Height. If you search on the net, there were so many reports with different scenarios of falling from height which results to serious injuries and death. Most of the reports are fall from height in which elevator or lift shaft were left open or unprotected inside the construction building.

"Elevator shaft falls are deadly, but it is preventable."

How to prevent these lift shaft falls?

Comply the OSHA fall protection standards.
Main contractors shall design a fall protection system to prevent or reduce the risk of serious or fatal accidents.
Conduct toolbox talks to workers & sub contractors of fall protection, work at height and highlight to them the possible hazards and how to identify prior working. Train them to recognize and avoid unsafe conditions.
Provide the workforce with correct PPE (Personal Protective Equipment). In this case - the fall arrest sytem - and train them how to wear and use it properly.
Conduct regular safety inspections throughout the construction building to ensure that the workplace are in safe condition.
Ensure all workforce "know-what-to-do" during emergency. This is very important. Also, develop an emergency plan that reflect the response time from local emergency services.

Example of Best Practice:

Here, you will find the elevator or lift shaft is fully protected with custom built and lockable lift shaft with protected cages. Workers who are working near the area will be protected from falling into the elevator shaft and the same working inside the shaft will be protected from falling materials. This must be done for all open lift shaft inside the construction building.

Important Note: The key to the door must be kept only by the lift superintendent and the main contractor. The steel cage will be fitted throughout the construction stage and to be removed only by the time the permanent lift doors will be installed and secured.
